APPROVED: The plan and specifications are approved based on the information provided to our office
for review. Approval is contingent upon meeting all of the requirements listed below. It is the project
owner's responsibility to retain the plans at the project location.
SCOPE: This report covers the project design's effect on the safety and sanitary quality of the drinking
water. This report applies to this project only, and not to any additional parts of the water system. The
examination of plans is based on the assumption that the information provided is accurate and
complete and that the owner has the necessary legal authority to construct the project. The project
designer is responsibile for, but not limited to, the structural features, material selection, sizing of
materials and equipment, equipment effectiveness and installation methods.
Plans and specifications are examined with regard to the location, construction, and operational
features of the design and maintenance of all parts of the system, which may affect the safety and
sanitary quality of the water. Examination is based on the standards of this department.
COMPLIANCE: Construction must be according to the approved plans and specifications. If changes are
made to the approved plans, Minnesota Department of Health (MDH) approval must be obtained before
such changes are made. Otherwise, the construction will be considered installed without proper
approval. A separate correspondence will be issued for changes or addenda.
INSPECTIONS: It is the responsibility of the owner or owner's agent to contact MDH for any inspections
that are listed in the requirements of this report.
This project consists of the installation of the following watermains:
1. 76 lineal feet of 6-inch C900 PVC watermain
2. 2,632 lineal feet of 8-inch C900 PVC watermain

Requirement(s):
1. It is understood that the installation will be in accordance with previously approved City Engineers
Association of Minnesota Watermain Specifications, 2018 edition.
2. A minimum vertical separation of 18 inches is required at all watermain and sewer main (house,
storm, sanitary and force) crossings. One full length of water pipe shall be centered at the point of
crossing so both joints will be equidistant and as far from the sewer as possible.
3. Hydrant drains shall not be connected to or located within 10 feet of sanitary sewers, storm sewers,
sewer catch basins or sewer manholes.
4. Hydrants placed where the groundwater table is above the drain hole outlet shall have the drain
hole plugged and be equipped with a tag stating the need for pumping after use.
The approval for construction of this project is valid for 2 years from the date of this letter. If project
construction has not begun within the two-year period, plans and specifications must be resubmitted to
MDH for approval before any construction takes place. While the project has been approved, additional
recommendations or requirements may be made to the project when changed conditions, additional
information or advanced knowledge make improvements necessary.
