I hereby apply for the above consideration and declare that the information and materials submitted with this application are
in compliance with City Ordinance and Policy Requirements and are complete and accurate to the best of my knowledge.
1 understand that the application will be processed for the next available meeting agenda after review of the information
submitted to determine if any other data is needed and after completion of a staff report.
I understand that all City incurred professional fees and expenses associated with the processing of this request are the
responsibility of the property owner and/or applicant and should be promptly paid. If payment is not received from the
applicant, the property owner acknowledges and agrees to be responsible for the unpaid fee balance either by direct payment
or a special assessment against the property.
(If the property fee owner is not the applicant, the applicant must provide written authorization by the owner to make

Sixty (60) Day Waiver: The City hereby notifies the applicant that development review may not be completed xdthin the

Main Ave. N.E., Albertville, Minnesota, to pass upon the proposed assessment of costs
related to sewer access charges for the Country Inn &. Suites Motel. The property
proposed to be assessed is Lot 6, Block 1 of the Shoppes at Towne Lakes- One plat in the.
City of Albertville...
The amount to be specially assessed against your particular lot, piece or parcel of
land (PID #101-099-001060) is $29,584.00. You may„at any time prior to certification of
assessment to the County Auditor, pay the entire assessment on such property to the City
Treasurer with no interest accrued to the date of payment. No interest shall be charged if
the entire assessment is paid within 30 days from the adoption of this assessment. You may
at ,any time thereafter pay to .the County Auditor the entire amount of the assessment
remaining unpaid, with interest accrued to December 31 of tie next succeeding year. If you
decide not to prepay the assessment before the date given above, the rate of interest tha~will
apply is 4.93% per. year. The right to partially prepay the assessment has not been
authorized by City ordinance. The City is proposing to spread the assessments over a four
year period. -
The proposed assessment roll is on file. for public inspection in~ the City. Clerk's

of the proposed assessment by the City Council maybe taken at the hearing. Written or oral
objections will be considered at the meeting. No appeal may be taken as to the amount of

City Administrator prior to the hearing or presented to the presiding officer at the hearing.

An owner ma_y, appeal an assessment to>the District Court pursuant to Minnesota
Statutes. Section 429:081 by serving written notice of the appeal upon the Mayor or City
Administrator within thirty (30) days after the adoption of the assessment and filing such
notice with the District ,Court .within ten (10) days after service upon the Mayor or City
Administrator.
Minnesota Statutes Section 435.193 through 435.195 authorize a municipality to
defer the payment of assessments against homestead property owned by persons 6S years of
age and older, or who are retired because of permanent and total disability under
circumstances where it would be a hardship for such person to make the assessment
payments. When deferment of the special assessment has been granted and is terminated for
any reason provided i~ that law, all amounts accumulated plus applicable interest become
k due. Any assessed property owner meeting the requirements of this law may, within 30
days of the confirmation of the assessment, apply to the City Administrator for the
prescribed form for such defertal of payment of this special assessment on said owners
