The right-of-way permit is approved with the following conditions:
1. Applicant shall provide a Construction Performance Bond in the amount of$I.200.00. The
form of security shaII be in accordance with the City of Albertville's Right-Of-Way
Ordinance.
2. AIl roadway, alleyway, aod driveway crossings shaII be cons1ructed by meaos oftrenchless
technology. No excavation sbaIl take place within aoy City street.
3. For established boulevards, the Applicant shall utilize trechless technology. Any disturbed
turf shaII be restored with 4" of topsoil aod MnDOT type lawn sod.
4. For non-established boulevards, the Applicaot may use open-cut or trenching methods for
installation. AIl disturbed areas shaII be restored with 4" of topsoil, seed, fertilizer, aod
mulch.
5. Forturfrestoration on slopes greaterthao 3:1, the Applicant shaII utilize stapled sod or the
combination of erosion control blanket, fertilizer aod seed.
6. AIl work shaII take place within designated utility easements aod/or City Right-Of-Way.
7. The Applicant shaII submit their Gopher One Call registration certificate number to the City.
8. The Applicant shall submit a Certificate of lnsuraoce in accordaoce with the City of
Albertville's Right-Of-Way.
9. The Applicant shaII re-establish all erosion controYsediment devices disturbed during utility
installation.
10. The security shall be retained until the City has been contacted aod conducted a final walk-
through aod all restoration items have been completed.
